BAYADA 居家护理项目配备 50 名 CRRN,将康复护理延伸至患者家中

This Nursing Credential Is Rare Even in Rehab Facilities. BAYADA Has Fifty.

As hospitals face pressure to safely discharge patients with complex conditions, research has linked this specialty to shorter hospital stays—and BAYADA is bringing it home.

, /PRNewswire/ -- As health systems face growing pressure to shorten stays and reduce readmissions, a small, highly specialized group of nurses is playing an outsized role in safely discharging people with physical disability and chronic illness.

BAYADAbility™ brings the expertise of Certified Rehabilitation Registered Nurses to clients with spinal cord injuries, traumatic brain injuries and other complex conditions recovering at home.

Certified Rehabilitation Registered Nurses, or CRRNs, hold a national credential earned by fewer than 1% of nurses, with specialized training in helping people with spinal cord injuries, traumatic brain injuries, and other serious, long-term conditions to regain function and independence. Research cited by the Association of Rehabilitation Nurses found that for every 1% increase in CRRN certification on a nursing team, patient length of stay dropped by approximately 6%.

BAYADA's CRRN-Led Home Nursing Program

BAYADA Home Health Care now employs 50 CRRNs in its BAYADAbility™ rehabilitation program—the first of its kind in home care since 1998—that enables people with long-term disability to receive high-quality rehabilitative nursing care in the comfort of home. To help more people in need, BAYADA is expanding this model of care into more communities by working with the right health system and payer partners.

What a CRRN Does Differently

General nurse training centers on treating acute conditions and managing symptoms. CRRN training focuses on something different: helping a person regain as much function and independence as possible after a serious injury or diagnosis. In practice, CRRNs are trained to:

  • Create personalized home care plans, connecting functional abilities, risks, and physical environment to specific recommendations for safety, comfort, and independence.
  • Guide safe transitions home, coordinating with discharge planners and case managers to prepare the home setting before a client leaves the hospital or rehab facility.
  • Determine safe mobility and equipment strategies so clients can move and act safely and independently in their own home and community.
  • Identify diagnosis-specific risks early, like pressure injuries, infections, mobility-related decline, and intervene before they lead to a hospital readmission.
  • Train the full care team, including local nurses and family caregivers, on diagnosis-specific techniques so care stays consistent around the clock.
  • Ensure safe policies and practices for clients, caregivers, and employees.

A CRRN's Impact, in One Client's Story

In March 2018, Howard Boone Jr. was 23 years old, an Army ROTC cadet one year from graduating college, when a stray bullet struck him in the neck and spinal cord. The injury left him paralyzed from the neck down.

Boone came home to Raleigh, North Carolina, later that year with a BAYADA care team and CRRN-guided clinical oversight. Today, at 32, he has not been hospitalized in nearly five years—an outcome his care team attributes to strict adherence to a CRRN-developed care plan addressing his highest risks: pressure injury prevention, infection control, and a consistent repositioning schedule.

That stability has given Boone room to build an extraordinary life. He graduated college. Using voice-controlled technology on his phone, he has written and published two books of poetry, hosted a podcast, and organized community events on gun violence prevention—all from home, supported by a nursing team he has known for years.

"I remember meeting Howard's mother at the inpatient rehab facility before he came home. She grabbed my hand and asked, 'You're here for me?'" said Dr. Stacey Rice, DNP, RN, NPD-BC, CCM, CRRN, BAYADA Director of Clinical Strategy and Innovation, who served as Howard's transitional care manager. "That is what a safe transition is all about. Nine years later, it is amazing what he has accomplished, and it is a reminder that more people and more communities deserve this level of rehabilitative home nursing care, too."
