Saleem joined ChronicCareIQ in May 2023 as Vice President of Customer Success and has overseen the company's operations as Senior Vice President since 2024.

As COO, Saleem will lead ChronicCareIQ's day-to-day operations, including customer success, implementation, and support, and he will work closely with the company's commercial and technology leaders to build the teams and processes behind the company's continued growth. He will also oversee the launch of a standalone product and innovation division, bringing together product management and a newly created Innovations and Solutions department so ChronicCareIQ can respond quickly and creatively to emerging needs and trends in care management.

Saleem has more than 15 years of experience scaling customer operations in healthcare technology. He began his career at athenahealth, where he spent more than seven years rising from Project Associate to Senior Manager, leading implementations for physician practices. He later served as Vice President of Client Services at Azalea Health, where he grew the services organization and launched new revenue lines. He went on to Canvas Medical as Vice President of Customer Operations before joining ChronicCareIQ in 2023.
